If you have a low income, the NHS Low Income Scheme could help you pay for;
- NHS prescriptions
- NHS dental treatment charges
- the cost of sight tests, glasses and contact lenses
- the cost of travelling to receive NHS treatment
- NHS wigs and fabric supports
The amount of help you are entitled to, depends on your household income and outgoings. You can apply for a certificate to receive free or reduced cost treatment. All you need to do is print out and complete an HC1 Application Form below or you can get a hard copy from our main reception and post it to NHS Low Income Scheme, NHS Business Services Authority, Bridge House, 152 Pilgrim Street, Newcastle upon Tyne, NE1 6SN.
